Nurse Navigator Services

Nurse Navigators use a multi-disciplinary approach to monitor high-needs clients, identify actions required to manage their health care and direct clients to the right service, at the right time and in the right place to prevent re-admission to hospital.

The Nurse Navigator service provide a central point of communication and engagement to clients and their families to optimise care and coordination of services.
